Insurance
Advanced Eye Care, SC participates with Medicare, most Medicare Replacement plans, and most commercial insurance plans. Some of the commercial plans in which we participate include:
- Blue Cross Blue Shield PPO/HMO
- United Health Care PPO/HMO/POS
- Humana PPO/POS/HMO
- Aetna PPO/EPO/POS/HMO
- Cigna PPO/HMO/EPO/POS
- AARP
- VSP

We want your visit to be smooth and stress-free. Here is a summary of what you need to know about insurance, payments, and appointments at our practice.
Insurance
- Please bring your insurance card, photo ID, and accepted form of payment to every visit.
- Your insurance company decides what they will pay after your visit.
- Any amount not covered by insurance is your responsibility.
Payment
- We accept checks, credit cards, and debit cards only.
- Copays and charges for on-covered services are due at your visit.
- Once your insurance has processed your claim, we will let you know if you still owe a balance.
- You can pay online or by check, credit, or debit card.
- Returned or declined payments may have a $30 fee.
Refractions
- The refraction test is usually not covered by insurance.
- The cost is $55 and due at your visit.
- We will bill your insurance and refund you if your insurance covers this test.
Referrals and Authorizations
- Some insurance plans require a referral or prior authorization. If needed, please make sure it is in place before your appointment.
- If it is missing, you may be responsible for the cost or the appointment may need to be rescheduled and a Missed Appointment Fee will apply.
Self-Pay Patients
- If you do not have insurance or choose not to use it, payment is due at the time of service.
- An estimate will be prepared for you.
Contact Lens Services
- Contact lens fittings and training have additional fees.
- A card on file and Autopay is required for all contact lens services.
Procedures and Surgery
- A card on file and Autopay is required for procedures and surgery.
Missed Appointments
- If you miss your appointment or cancel with less than 24 hours’ notice, there is a $50 fee.
Medical Records and Forms
- Your medical record is available at no cost through the patient portal.
- Administrative or legal forms may have a $31 fee.
Past Due Balances
- Balances are due when billed.
- Accounts that remain unpaid may be sent to collections and may result in discharge from the practice.
